Definition

Occupational therapists can optimize and prolong an older driver's ability to drive safely and ease the transition to other forms of transportation if driving cessation becomes necessary. By identifying strengths as well as physical or cognitive challenges, occupational therapists can evaluate an individual's overall ability to operate a vehicle safely and recommend assistive devices or behavioral changes to limit risks. Occupational therapy practitioners offer a continuum of services related to community mobility, from evaluation of driving performance, through counseling and support for lifestyle changes, to maintaining independence and quality of life.

Where 225XR0403X sits

Choose 225XR0403X over the general Occupational Therapist code (225X00000X) when the provider's training or certification is specifically in driving and community mobility; the general code covers the classification as a whole. The code goes in the taxonomy field of the NPI application in NPPES, and on claims where a payer asks for it — box 24J (shaded) or 33b on the CMS-1500 with the ZZ qualifier, or the PRV segment of an 837 electronic claim. Payers match it against the taxonomy on the NPI record and, for Medicare, against the specialty on the enrollment, so all three should agree. To see which code a provider uses, search their NPI in the lookup.
