A neurological surgeon treats patients with disorders of the nervous system; disorders of the brain, meninges, skull, and their blood supply, including the extracranial carotid and vertebral arteries; disorders of the pituitary gland; disorders of the spinal cord, meninges, and vertebral column, including those which may require treatment by spinal fusion or instrumentation; and disorders of the cranial and spinal nerves throughout their distribution.

Where 207T00000X sits

Neurological Surgery has no specializations, so 207T00000X is the only code for it. A provider may report more than one taxonomy code on an NPI but marks one as primary.

Medicare

CMS's taxonomy crosswalk links 207T00000X to Medicare specialty 02 (Physician/General Surgery) and Medicare specialty 14 (Physician/Neurosurgery). That specialty — not the taxonomy code itself — is what Medicare enrollment records and uses for payment. 27,817 practitioners are enrolled under it nationally, and 24 other taxonomy codes map to the same specialty.

Other codes under the same Medicare specialty: 204E00000X Oral & Maxillofacial Surgery (D.M.D.); 204F00000X Transplant Surgery Physician; 207X00000X Orthopaedic Surgery Physician; 207XP3100X Pediatric Orthopaedic Surgery Physician; 207XS0106X Orthopaedic Hand Surgery Physician; 207XS0114X Adult Reconstructive Orthopaedic Surgery Physician; 207XS0117X Orthopaedic Surgery of the Spine Physician; 207XX0004X Orthopaedic Foot and Ankle Surgery Physician; and 16 more.

The code goes in the taxonomy field of the NPI application in NPPES, and on claims where a payer asks for it — box 24J (shaded) or 33b on the CMS-1500 with the ZZ qualifier, or the PRV segment of an 837 electronic claim. Payers match it against the taxonomy on the NPI record and, for Medicare, against the specialty on the enrollment, so all three should agree. To see which code a provider uses, search their NPI in the lookup.
