Definition

Rehabilitation is the discipline focused on restoring a patient's functional abilities to pre-injury or pre-disease status. Functional abilities are defined as those activities in one's daily life, work, or sports and recreational activities that an individual participates in. Relevant impairments (e.g. strength, endurance, flexibility, motor control, etc.) are often intermediate goals of rehabilitation, but the final goal of successful care is return to participation in activities in which the patient was successful before the onset of the injury or disease. Essential to a rehabilitation approach is a focus on patient-centered outcomes such as independence and self-management or self-care skills.

Where 111NR0400X sits

Choose 111NR0400X over the general Chiropractor code (111N00000X) when the provider's training or certification is specifically in rehabilitation; the general code covers the classification as a whole. A provider may report more than one taxonomy code on an NPI but marks one as primary.

The code goes in the taxonomy field of the NPI application in NPPES, and on claims where a payer asks for it — box 24J (shaded) or 33b on the CMS-1500 with the ZZ qualifier, or the PRV segment of an 837 electronic claim. Payers match it against the taxonomy on the NPI record and, for Medicare, against the specialty on the enrollment, so all three should agree. To see which code a provider uses, search their NPI in the lookup.
